Commitment & Safety Agreement Form

This document aims to ensure the smooth running of the workshop and the safety of all participants within the workshop, lounge, and garden areas.

1.Participant Information

2. Health Status & Fitness

• I certify that I have no medical contraindications to participate in the proposed activities.

• I commit to reporting any physical discomfort during the relaxation exercises.

3. Safety & Responsibility Commitment

• Outdoor areas: I acknowledge that the garden is a natural space. I commit to staying on designated paths and exercising caution (wet or uneven ground).

• Craft tools: I commit to handling feathers, inks, and sharp tools with care, following the instructor's guidelines.

• Personal belongings: I remain fully responsible for my valuables. The organization declines all responsibility in case of loss or theft.

• Property damage: The workshop involves the use of indelible inks. I acknowledge that I have been advised to wear appropriate clothing (or an apron).

4. Code of Conduct

• I commit to respecting the peace and quiet of the workshop, punctuality, and the digital disconnection policy (phones on silent mode).

• Media rights: I authorize the organizer to photograph my creations for promotional purposes (social media, website).
